Licensed Clinical Social Worker

Provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities; assess residents, develop and follow treatment plans, and coordinate with a multidisciplinary team and facility staff. Document treatment plans and psychotherapy sessions, maintain HIPAA compliance and confidentiality, and communicate with primary care physicians, facility staff, and support personnel to ensure medically necessary care. Maintain professional conduct and adhere to relevant policies, procedures, and regulatory guidelines. Knowledge and application of assessment and psychotherapy techniques, DSM guidelines, and effective documentation in EMR systems are required.
