Job Summary
Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) to provide recovery-oriented rehabilitation services in group or individual settings for adults in skilled nursing and assisted living facilities. Conduct assessments with valid consent, develop and follow treatment plans, coordinate with primary care physicians and facility staff, and document plans and sessions within required timelines. Maintain HIPAA compliance, communicate with facility staff and clinical supervisors, and adhere to regulatory and ethical standards. Requires MSW and current state license; collaboration within a multidisciplinary team to improve quality of life for residents. A signing bonus of up to $5,000 is offered. Strong knowledge of assessment and psychotherapy techniques, DSM, confidentiality, and effective written and verbal communication is expected.
Required Qualifications
- Master’s degree in social work
- Current license to practice in the state
- Licensed Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) in good standing
