Licensed Clinical Social Worker

Licensed clinical social work professional providing recovery-oriented rehabilitation services in long-term care settings (skilled nursing and/or assisted living). Responsibilities include assessing residents with valid consent, developing and following treatment plans to meet individual needs, documenting treatment plans and psychotherapy sessions, and providing written updates for the facility medical record within 48 hours. Work involves consultation with primary care physicians, family, facility staff, and Eventus WholeHealth support staff; maintaining open communication with facility staff and administration; adhering to Eventus WholeHealth standards, procedures, HIPAA rules, and regulatory guidelines; and consulting with the clinical supervisor as needed. Required knowledge includes assessment and psychotherapy techniques, DSM-based diagnostics, and regulatory rules governing psychotherapy practice. Skills include gathering and analyzing physiological, socioeconomic, behavioral, and emotional data; evaluating problems and recommending appropriate interventions; strong written and verbal communication; initiative, judgment, discretion; and maintaining effective working relationships. Abilities include staying calm in emergencies, collaborating with health-care team members, making responsible decisions within scope of practice, and contributing to continuous quality improvement. The role is with Eventus WholeHealth, focused on residents in facilities in Raleigh, NC.
