Job Summary
Provide individual, family, and group therapy to long-term care residents while conducting clinical assessments, psychosocial evaluations, and treatment planning. Support patients with depression, anxiety, grief, trauma, and substance use through behavioral management and goal-oriented interventions aligned with geriatric best practices. Collaborate with nursing, social services, and medical providers to coordinate care plans and participate in facility meetings. Maintain complete documentation using the Geriatric Notes Tool and adhere to ethical and regulatory guidelines. Build professional relationships with facility staff and leadership to serve as a trusted behavioral health resource. Work onsite in local long-term care facilities with a predictable Monday–Friday schedule, no evenings or weekends, and no on-call rotation.
Required Qualifications
- Active, unrestricted LCSW license
- Valid driver's license and reliable transportation
- Strong written, verbal, and interpersonal communication skills
- Ability to work independently while collaborating with an interdisciplinary team
- Comfortable navigating LTC environments
- Commitment to timely documentation and high-quality care
