Licensed Clinical Social Worker

Provide individual, family, and group therapy to long-term care residents while conducting clinical assessments, psychosocial evaluations, and treatment planning. Deliver behavioral management and brief, goal-oriented interventions for conditions such as depression, anxiety, grief, and substance use. Collaborate with nursing, social services, and medical providers to coordinate care and participate in facility meetings and case discussions. Maintain complete documentation using the Geriatric Notes Tool in adherence to ethical and regulatory guidelines. Build professional relationships with facility staff to serve as a trusted behavioral health resource. Work onsite in local communities with a predictable Monday–Friday schedule, no evenings or weekends, and full administrative support.
