Job Summary
Provide bio-psycho-social assessments, care coordination, and clinical interventions within the Medical Social Work department. Conduct program development and discharge planning; intervene with medically and psychosocially complex patients, ensuring alignment with Cottage Health's Patient First Philosophy and organizational Mission and Vision.
Required Qualifications
- Master's in Social Work (MSW) from CSWE-accredited School of Social Work
- Current California Clinical Social Work License (LCSW) required upon hire; CCS paneling within six months of hire
- Preferred: CCS paneling within six months of hire; Certified Social Worker in Health Care (C-SWHC) optional
- Minimum 1-2 years of relevant clinical social work experience; Preferred 3-4 years in healthcare setting
