Job Summary
Conduct advanced clinical social work practice in a medical setting, applying social work theory and ethics to restore psychosocial functioning for individuals, families, and communities. Perform prevention, assessment, and treatment of mental, emotional, behavioral, and addictive disorders while engaging in multi-disciplinary treatment planning and coordinating complex discharge plans. Interview and assess patients, identify helpful resources, maintain patient records, and communicate critical information to the medical team. Participate in an on-call rotation and adapt behavior to specific patient populations while complying with all policies, standards, and regulatory requirements.
Required Qualifications
- Master's in social work (MSW)
- 5 years of related experience
- Current Licensed Clinical Social Worker (LCSW) in the state of practice and able to engage in independent practice
- Licensure or certification as a sex therapist
- Good knowledge of common disease entities and disorders and medical terminology
- Advanced knowledge of an acute and chronic care medical setting, medical terminology and the psychosocial and economic effects of illness and/or trauma upon the patient and their support system
- Advanced knowledge of managed care and public programs such as those funded by Medicare, Medicaid, Medicare Advantage, and commercial insurance plans
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process
- Ability to lift 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly
