Licensed Clinical Professional Counselor

Provide individual, family, and group therapy to long-term care residents while conducting clinical assessments, psychosocial evaluations, screenings, and treatment planning. Deliver behavioral management and brief, goal-oriented interventions for conditions including depression, anxiety, adjustment to illness, grief, trauma, and substance use. Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate treatment and participate in care plan discussions and facility meetings. Maintain complete documentation using the Geriatric Notes Tool in adherence to ethical and regulatory guidelines. Build professional relationships with facility staff and leadership to support residents' mental and emotional needs. Work onsite in local long-term care facilities with a predictable Monday–Friday schedule, no evenings or weekends, and no on-call rotation.
