Job Summary
Clinical role providing Therapeutic Behavioral Services and case management for residents in long-term care (skilled nursing/assisted living). Responsibilities include developing individualized treatment plans, coordinating behavioral health and healthcare supports, providing crisis intervention and de-escalation training to staff, facilitating group or individual services, maintaining communication with facility staff and residents' families, and adhering to HIPAA and facility standards. Requires a Master’s degree in counseling or marriage and family therapy and eligibility for licensure in the practicing state; licensure as an LPC or LMFT is expected, with emphasis on delivering recovery-oriented care within an interdisciplinary team and ensuring quality and regulatory compliance.
Required Qualifications

- Eligibility for license to practice in the state of practice
