Job Summary
Provide Therapeutic Behavioral Services and case management to adults in skilled nursing and long-term care facilities as part of an interdisciplinary team. Partner with licensed clinicians to develop individualized treatment plans, coordinate healthcare and behavioral health services, deliver crisis prevention and daily life-skills training, and support residents and their families. Responsibilities include staff training on triggers and de-escalation, maintaining professional communications with facility staff, adhering to regulatory and ethical standards, and ensuring patient confidentiality within HIPAA guidelines. Opportunities may arise for future transition to direct counseling services within the same multidisclipinary care setting.
Required Qualifications
- Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) or equivalent
- Master’s degree in counseling or marriage and family therapy
- Eligibility for licensure in the practicing state
- Knowledge of behavioral health practices and HIPAA
- Clinical documentation skills
- Ability to work with residents in long-term care settings
- Strong communication and teamwork abilities
