Licensed Certified Social Worker, LCSW - Home Health

Conduct home visits to clients in Essex and eastern Middlesex County territories to provide clinical social evaluations and plan interventions. Identify community resource referrals to address practical and environmental needs, then act as an advocate to navigate patients through the community system. Participate in developing total plans of care with multidisciplinary clinicians and maintain contracts with public and private agencies. Educate patients and family members on resources to promote health, safety, and independent living while documenting observations and evaluations on a tablet. This per diem role offers $60-70 per point with prior home health experience preferred.
