Licensed Certified Social Worker - Clinical (LCSW-C)

Assess psychosocial needs, strengths, and support networks of patients with mental, behavioral, and emotional issues. Develop care plans and treatment goals while providing individual and group therapy to help patients solve life problems. Establish rapport, advocate for patients, and coordinate services for behavioral health concerns. Serve as a liaison to promote the health center to the behavioral community and assist primary care physicians. Pursue additional grant and funding opportunities to expand the center's capacity to meet community health needs. Full-time or part-time onsite work at the Conowingo, MD office.
