Job Summary
Conduct individual, group, and family counseling to assist patients and families in achieving treatment goals while facilitating weekly group therapy sessions. Perform initial assessments by the close of the first business day following admission and complete ongoing assessments during informal and formal sessions to track progress toward goals. Collaborate with the interdisciplinary team, psychiatrists, and community providers to develop individualized treatment plans and manage discharge planning, ensuring transitions between levels of care are seamless. Provide milieu management and admission assessments during low census periods and utilize CPI methods when controlling aggressive patients. Document all interactions timely, succinctly, and in the DAIR format, while ensuring patient rights are protected in accordance with state laws and hospital policies.
Required Qualifications
- Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Licensed Clinical Professional Counselor (LCPC) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Crisis Prevention Institute (CPI) Training within 3 months
- Master's Degree: Counseling; Psychology; Social Work
- 1+ years of experience
- Advanced reading skills

- Ability to respond appropriately to customer/co-worker
- Interaction with a wide variety of people
- Maintain confidential information
- Ability to communicate only the facts to recipients or to decline to reveal information

- Basic computer knowledge: Uses word processing, spreadsheet, e-mail application, and web browser
- Comfortable within a Windows OS and learning new applications
- Use of usual and customary equipment used to perform essential functions of the position
- Work may occasionally require travel to other Carle Health facilities/hospitals
- Required to drive your own vehicle for business purposes
- Demonstrates thorough knowledge of the Mental Health Code, the Confidentiality Act, The Mental Health Treatment Preference Act, HIPAA, laws pertaining to guardianship and custody, advance directive legislation and MMCI policies, as well as Elderly and Child Abuse reporting laws
- Assists patients/families/staff in obtaining temporary or permanent guardianship whenever necessary
- Assists with appropriate filing of petitions, certificates, etc. with the court
- Makes necessary phone calls to child and elderly welfare agencies to report abuse and/or neglect
