Job Summary
Provide clinical recommendations and training to providers, caregivers, and family members while assisting with the development and monitoring of Person-Centered, Behavior Support, Mental Health Treatment, and Crisis Safety Plans. Complete agency and individual assessments to guide clinical interventions, facilitate rapid referrals to prevent crisis escalation, and establish collaborations with provider agencies, payers, and community stakeholders. Deliver best-practice protocols through consultation and training, including in-person or virtual sessions on crisis prevention measures, and provide clinical supervision to mental health practitioners and interns. Maintain accurate record-keeping integrated with EHR systems to facilitate reimbursement and report suspected abuse in accordance with state regulations. Serve as a member of the Active Consulting Team in LA County, CA, supporting individuals with IDD and mental health diagnoses at risk of inpatient placement or law enforcement involvement.
Required Qualifications
- Current California Licensure as LCSW/LPC/LMFT
- Maintain professional license in good standing with applicable licensing board(s)
- Valid BLS CPR and Basic First Aid certification
- Crisis de-escalation verbal skills and physical management training certificate, i.e., Crisis Prevention Institute (CPI) certification
- Two years' experience providing case management services to individuals with IDD and/or Behavioral Heath diagnoses
- Valid driver's license and auto insurance
- Must be computer literate
Desired Qualifications
- Fluent bilingual speaker in one or more of the following languages: (English/Spanish; English/Mandarin; English/Korean; English/Tagalog)
- Two years' experience working with individuals diagnosed with IDD and behavioral and mental health diagnoses
- Experience working with individuals in crisis in community settings, i.e., residence, school, jobsite, etc.
- Working knowledge of the California DDS, MHSD and Regional Centers service systems
