Job Summary
Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team. Perform services in accordance with the physician's orders and the established plan of care. Maintain a valid state driver license, automobile liability insurance, and dependable transportation, while holding current CPR certification for healthcare providers. Prioritize and handle multiple critical tasks simultaneously in a high-stress environment, communicating effectively both orally and in writing. Make independent decisions relevant to patient needs while functioning well amidst interruptions.
Required Qualifications
- Bachelor's degree from a school of social work accredited by the council on social work education
- Certification or licensure in the state where they currently practice
- At least one year of social work experience in a health care setting
- Basic demonstrated technology skills, including operation of a mobile device
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- Ability to safely drive an automobile in all types of weather conditions
- CPR certification for the healthcare provider
- Organized and detail-oriented
- Ability to communicate effectively both orally and in writing
- Capability of making independent decisions relevant to the patient's needs
- Capability of prioritizing and handling multiple tasks of critical importance simultaneously
- Ability to function well in atmosphere of stress and interruption
Desired Qualifications
- Bachelor's degree in psychology, sociology, or other field related to social work
- Certification or licensure in the state where they currently practice
- Medicare home health or hospice experience
