Job Summary
Assist with health assessments, wellness monitoring, and medication administration in accordance with physician orders. Support the development of individualized care plans and promote health education and preventative care practices. Collaborate with registered nurses, physicians, specialists, families, and interdisciplinary team members to ensure coordinated care while maintaining a safe, person-centered environment for participants. This role offers a consistent day shift schedule with no nights, weekends, or overnight rotations. Bilingual candidates fluent in Cape Verdean are encouraged to apply. Required Qualifications
- Bilingual candidates, especially those fluent in Cape Verdean
- Graduate of an accredited Licensed Practical Nurse (LPN) program
- Current Massachusetts LPN license
- Current CPR and First Aid certification
- Strong communication, organizational, and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
