LHCSA RN Care Services Supervisor- Monroe County

Coordinate quality patient care and manage case loads for home health aides and personal care aides, conducting admission assessments and developing comprehensive plans of care. Perform in-home supervisory visits to oversee clinical skills, provide mentorship to new hires, and execute the Home Health Aide Training Program from classroom to field. Maintain accurate documentation, communicate with MLTC case managers and physicians, and address staff concerns or incidents per regulatory guidelines. Ensure annual performance evaluations and in-service credits are completed while establishing relationships with community partners. This role requires a current NYS RN license, two years of home care experience, and a Bachelor's degree preferred.
