Job Summary
Provide day-to-day care and support to individuals with learning disabilities to enable them to live as independently as possible. Deliver personal care including bathing, dressing, and medication management, while assisting with housekeeping tasks like cooking and laundry. Promote independence, wellbeing, and rights by facilitating access to community services and supporting goal achievement within person-centred care plans. Maintain high-quality service provision by ensuring accurate record keeping, completing risk assessments, and complying with CQC and internal safety policies. Liaise with external professionals such as GPs and social services to support care pathways. Participate in service user reviews, contribute to continuous improvement initiatives, and assist in the induction of new colleagues.
