Lead Youth Family Foundations Housing Case Manager

Job Summary

Train and supervise Youth Family Rehousing Case Management staff while receiving, assigning, and monitoring new referrals to Youth Family Housing Programming. Develop a network of available services to assist participants and staff, support onboarding and program entry documents, and lead team meetings to address goals. Collaborate with housing providers and partners to establish working relationships, provide oversight of scattered site/residential programming, and manage data input and case management software. Uphold organizational values of Equity, Integrity, and Compassion while managing multiple priorities in a fast-paced environment. This role supports young adults (14-25) and their families entering housing programming at the Youth Opportunity Center within Community Teamwork's Youth Services team.

Required Qualifications

  • High School diploma or equivalent education
  • 2+ years of experience working with youth and young adults
  • Ability to work comfortably with pregnant and parenting youth in both group and individual settings
  • Minimum of one-year of experience with assessment, advocacy and case management work in a Human Service field
  • Demonstrated knowledge of community resources available to low-income individuals and families
  • Demonstrated knowledge of supportive housing programming
  • Excellent customer service, organizational, oral and written communication skills
  • Ability to manage multiple priorities and work in a fast-paced environment
  • Proficient computer skills in basic computer software and data collection systems
  • Excellent crisis management skills
  • Ability to maintain accurate records and confidentiality of information
  • Excellent attention to detail
  • Valid Driver's License

Desired Qualifications

  • Bachelor's Degree, preferably in a Human Services related field
  • Minimum 5+ years' direct case management experience may substitute for degree
  • Experience with families
  • Bilingual skills (English/Spanish, English/Khmer, English/Portuguese)
