Job Summary
Lead Youth Family Foundations Housing Case Manager at Community Teamwork’s Youth Services team (Youth Opportunity Center). Responsibilities include overseeing advanced level case management, onboarding and program-entry support, supervising the Youth Family Foundations Housing team, coordinating referrals, developing a network of housing resources, facilitating team meetings, collaborating with program staff and partners, overseeing scattered-site/residential and rental assistance programming, maintaining relationships with housing providers, and handling data input and administrative duties. Requires strong advocacy, housing program knowledge, detailed record-keeping, client confidentiality, and bilingual capabilities preferred. Minimum High School diploma; 2+ years working with youth/young adults; driver’s license; preferred Bachelor’s degree in a Human Services field with 5+ years of experience possible in lieu of degree.
Required Qualifications
- High School diploma or equivalent education
- 2+ years of experience working with youth and young adults
- Minimum of one-year of experience with assessment, advocacy and case management work in a Human Service field
- Demonstrated knowledge of community resources available to low-income individuals and families
- Demonstrated knowledge of supportive housing programming
- Excellent customer service, organizational, oral and written communication skills
- Ability to manage multiple priorities and work in a fast-paced environment
- Proficient computer skills in basic computer software and data collection systems
- Excellent crisis management skills
- Ability to maintain accurate records and confidentiality of information
- Valid Driver’s License
