Lead Physician Primary Care Over 300k Potential Compensation

Lead Physician, Primary Care responsible for managing a panel of medically complex and socially vulnerable members within a team-based, population-health focused center. You will coach and mentor an interdisciplinary care team, provide high-quality primary care, lead clinical decision-making with the Medical Director, participate in leadership meetings, and serve as the center’s clinical leader and ambassador to external partners. Responsibilities include managing chronic and acute conditions, developing and adjusting patient care plans with behavioral health, nursing, social work, and other professionals; driving quality, utilization, and engagement metrics; implementing evidence-based, value-based care in a PCMH framework; overseeing transitional care plans and reducing ER visits and readmissions; and providing on-call coverage. Qualifications emphasize an active unrestricted medical license, 5+ years leading interdisciplinary teams, experience in clinic/community/inpatient settings, mentoring experience, strong problem-solving and project-management skills, and a passion for population health and care model development. Working hours typically Monday–Friday 8–5; location Hamilton County, OH.
