Job Summary
Oversee patient cohorts and conduct in-home and telehealth visits for Medicare Advantage members, leveraging Clover Assistant and EMR to deliver preventive care. Coordinate with interdisciplinary teams to create personalized care plans and close gaps in service. Provide clinical leadership by mentoring nurse practitioners, guiding professional growth, and ensuring high-quality delivery across the home care team. Advance clinical excellence through quality improvement initiatives, assurance reviews, and collaboration with the Medical Director to optimize care processes. Requires an MD/DO degree with board certification in Internal Medicine, Family Medicine, or Geriatric Medicine, plus five years of advanced clinical practice experience. Full-time hourly role with a base salary range of $220,000 to $240,000 USD, offering relocation support for Georgia-based candidates.
Required Qualifications

- Licensed in Georgia
- DEA registration
- CDS registration
- Minimum of five years of advanced clinical practice experience
