Job Summary
Lead Navigator for Community Bridges, Inc. in Arizona. Seasonal role (May–October) focused on outreach, engagement, and wrap-around supportive services for individuals experiencing homelessness and behavioral health crises. Responsibilities include coordinating entry, housing referral, leasing up, and discharge processes; mentoring Navigator II; working within a multi-disciplinary team to coordinate care with community providers. Requires recovery from AOD and/or GMH for at least 1 year, and prior Navigator/Peer Support experience (≥90 days). This role emphasizes outreach to high-acuity individuals, connecting them to RBHA and Non-RBHA services, and delivering lifesaving support during extreme heat events. Strong emphasis on empathy, community engagement, and responsiveness to urgent needs. Required Qualifications
- Highschool diploma or GED is required
- Associates Degree (or higher) in a field related to behavioral health is preferred but not required
- 1-3 years of full-time Health Care related work experience preferred but not required
- Current AZ Driver's License
- Clear 39-month Motor Vehicle Record
- Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
- Arizona Fingerprint Clearance card, site specific
