Job Summary
Coordinate team-based medical care for children in foster placement, serving as the primary liaison with health care providers to ensure timely and comprehensive services. Prepare, compile, and maintain accurate medical records while reviewing the ORR portal for compliance with information upload requirements. Establish partnerships with medical providers, provide updates to sponsors and families, and actively participate in resource development and discharge planning. Supervise staff, coach team members on medical protocols, and represent medical needs in planning, coordinating, and evaluating all health services for designated social service programs. Ensure compliance with California Community Care Licensing and public health department regulations while maintaining a safe, trauma-informed environment.
Required Qualifications
- 2+ years working experience in child welfare
- 1+ years of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care
- Licensed Registered Nurse (RN) with a Bachelor's
- Higher health-related qualification (e.g. Physician Assistant, Nurse Practitioner)
- Must be at least 21 years of age
- Valid Driver License
- Licensed Registered Nurse (RN) with a bachelor's degree or a higher health-related qualification (such as a Physician Assistant or Nurse Practitioner) and two to four years' experience working with children and families
- Bilingual in Spanish
- Comprehend and apply principles of advanced calculus, algebra and advanced statistical theory
- Apply logical thinking to a wide range of intellectual and practical problems
- Proficiency in MS Office (Word, Excel, Outlook)
- Proficient in the use of standard office equipment
- Electronic Medical Record and UC Portal Software
- Must be able to occasionally lift and/or move up to 10 pounds
- Specific vision abilities required by this job include close vision
