Job Summary
Conduct hybrid patient care and manage daily center operations, ensuring smooth patient flow and monitoring CMAP and HEDIS measures. Support clinical managers by supervising Care Promoters, training staff on health education and medication services, and overseeing quality activities including equipment checks and inventory management. Perform routine screening tests, prepare patients for exams, process phone messages, and call in prescriptions using the ChenMed Rx system. Maintain center safety through quality improvement initiatives while managing scheduling, wait times, and patient education. Requires an unrestricted LPN license, three years of clinical experience, BLS certification, and ability to work evenings, weekends, and holidays.
Required Qualifications
- High school diploma or equivalent education (GED)
- Unrestricted LPN license in state of employment
- IV Therapy certification if required by state
- A minimum of 3 years of work experience as an LPN in a hospital, medical clinic or comparable environment
- Prior Team Lead experience preferred
- Experience working with geriatric patients is a plus
- EMR system experience preferred
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
- Spoken and written fluency in English required
- Ability and willingness to travel locally, regionally and nationwide up to 20% of the time
- Flexible to work evening, weekends and/or holidays as needed

- Prior Team Lead experience preferred
- Experience working with geriatric patients is a plus
- EMR system experience preferred
