Job Summary
Lead Home Care Coordinator oversees onboarding of new clients, trains and supports caregivers during case transitions, conducts client discovery to tailor care plans, performs QA/QC home visits, and supports recruitment and administrative tasks. Responsibilities include developing care plans, ensuring client-specific care plans and expectations are understood, coaching caregivers in the field, maintaining documentation and timekeeping procedures, and providing ongoing communication with clients, families, and internal teams. The role combines client-care delivery (approximately 50% of time) with administrative and support duties, requires certifications (CNA or HHA), and travel to client sites in Connecticut and New York. Required Qualifications
