Lead Financial Care Manager

Job Summary

Manage a caseload of up to 20 higher-need clients and serve as a subject matter resource for Financial Case Managers regarding Medicaid, Social Security, and Medicare. Conduct direct client work by assessing financial situations, developing budget plans, and advocating with creditors and financial institutions. Support the team through onboarding, peer mentoring, and quality reviews of client files, applications, and billing practices. Review and approve monthly invoices, coordinate workflow to ensure timely completion of benefit applications and reports, and participate in generating standard operating policies. Meet with clients at least twice annually across residences, nursing facilities, and hospitals while maintaining accurate case records and metrics.

Required Qualifications

  • 2 years' banking or financial services experience
  • strong financial acumen
  • working knowledge of community resources
  • knowledge of the eligibility requirements and application processes of benefits and programs applicable to clients, including but not limited to SSI and Medicaid, MMLTC, VA, Medicare, SNT
  • attention to detail
  • ability to multi-task
  • ability to establish priorities and complete work in a timely manner despite changes in workload, deadlines, or competing requirements
  • Strong organizational, verbal, written and telephone communication skills
  • valid NYS Driver's License
  • Ability to drive tactical results to achieve strategic goals
  • Must demonstrate knowledge of the eligibility requirements and application processes of benefits and programs applicable to clients, including but not limited to SSI and Medicaid, MMLTC, VA, Medicare, SNT
  • Must have attention to detail and ability to multi-task
  • Skilled at establishing priorities and completing work in a timely manner despite changes in workload, deadlines, or competing requirements
  • Strong organizational, verbal, written and telephone communication skills required
  • Ability to maintain composure under strict deadlines and difficult situations
  • Ability to work in multi-cultural situations
  • Normal daily walking and sitting
  • Some lifting (files, etc.) <35 lbs
  • 50% of the work is community based: must be able to meet with individuals in their home environment and in a variety of community settings including hospital and nursing home environments
  • 50% of the work is performed at a desk or workstation

Desired Qualifications

  • Bachelor's degree in Business, Finance, Human Services, or a related field preferred
  • An Associate degree in Applied Science plus two years of relevant experience may be considered
  • Expertise in Medicaid Program and other benefit programs preferred
  • Prior experience working with and advocating for older adults preferred
  • Computer experience preferred (Word, Excel, EMS, Office 365, web research)
