Job Summary

Lead Community Health Representative promotes health and well-being through education, outreach, advocacy, prevention, and care coordination. Serve as a liaison between patients, families, providers, and community partners to support access to culturally appropriate services and coordinate care across medical, dental, behavioral health, and support programs. Mentor CHR staff, promote quality service delivery, and organize community health outreach events, including some weekends. Maintain patient confidentiality, work within a multidisciplinary team, and demonstrate deep respect for Native American cultural heritage.

Required Qualifications

  • A minimum of an associate's degree (or equivalent college credits) in Community Health and Human Services or related field
  • Certified Medical Assistant (CMA)
  • Certificate from CHR/CHW core competency training
  • Minimum of three (3) year's experience as a CHR/CHW
  • Maintain patient confidentiality
  • Ability to work well within a multidisciplinary team
  • Possess current CPR and First Aid certification or be willing to obtain within three (3) months of hire
  • Must maintain a valid, unrestricted California Driver License and ability to be insured
  • Must pass a pre-employment drug screen, physical and background investigation
  • Ability to work non-traditional hours including some weekends to assist supervisors to organize community health outreach events
  • Must provide a current DMV printout
  • All employees must be sensitive to the needs of the Native American community
  • demonstrate a deep understanding and respect for the diverse cultural heritage, traditions, and historical context of the Native American community

Desired Qualifications

  • Prior experience with providing community assistance in tribal specific programs and organizing health/wellness grants community events preferred
