Job Summary

Conduct personal assistance and health-related tasks per the Individualized Service Plan, maintaining a clean, safe, and comfortable environment while practicing infection control. Observe and report changes in client condition to the office, verify completion of daily tasks in WellSky, and ensure accurate documentation. Schedule caregivers to ensure full shift coverage, monitor daily schedules, and address open shifts by adding requested changes and communicating updates to the office and clients. Train and coach new caregivers, escalate performance concerns to the Market Director, and serve as a backup caregiver to ensure continuity of care. Meet with new clients to assess needs, conduct post-hospital visits, and educate families on available services.
