Lead Care Coordinator (LPN) coordinates services for patients with serious, complex, or chronic health problems, delivering preventive care, advocacy, information, and referrals. Oversees and coordinates Care Coordinator Team Lead functions, coordinating patient plans in collaboration with PCPs; maintains PHI confidentiality per HIPAA; requires strong data-analysis, documentation, and interpersonal skills, with proficiency in Microsoft Office and electronic records. 40 hours per week, located in Searcy, AR.
