Job Summary
Lead Case Manager role overseeing a team of case managers, providing supervision, caseload management, and coordination of services across healthcare, housing, mental health, and legal sectors. Manages assessments, develops and monitors individualized care plans, advocates for clients, ensures compliance and documentation, and drives quality improvement. Requires a Bachelor’s degree in a related field, 3-5 years of case management experience, QIDP certification or eligibility, Illinois driver’s license, and strong leadership, communication, and organizational skills.
Required Qualifications
- Bachelor’s degree in Social Work or related field
- Minimum 3-5 years of experience in case management or related field
- QIDP certified, or eligible to obtain certification within first three months
- Valid Illinois driver’s license with safe driving record
- Current proof of automobile insurance
- Strong leadership and supervisory abilities
- Excellent communication, organizational, and interpersonal skills
- Knowledge of laws, regulations, and industry standards
- Proficiency in case management software and Microsoft Office Suite
- Strong problem-solving, conflict resolution and decision-making skills
