Job Summary
The Lead Case Manager for Intensive Case Management Services (ICMS) focuses on assisting vulnerable populations in transitioning from homelessness to permanent housing through case management. Responsibilities include assessing client eligibility, developing individualized service plans, conducting psychosocial re-assessments, and providing transportation and support for housing acquisition. The role requires collaboration with mental health and health agencies, monitoring client wellbeing, and engaging with landlords for housing opportunities. Candidates should have experience with the chronically mentally ill and homeless, possess strong communication skills, and have a valid driver's license.
Required Qualifications
- BA Degree preferred
- Two years related experience in social services or related field
- Possess a valid California Class C Driver License
- Maintain an insurable driving record and safe vehicle
