Job Summary
The Lead Case Manager/Housing Navigator provides leadership and supervision to a team responsible for housing-focused support services. Key responsibilities include overseeing case managers and resource navigators, assessing needs for re-integration programs, developing employment training assistance, and building community partnerships. The role demands strong communication skills, competence in case management practices, and experience working with unhoused and addiction recovery populations. A Master's in social work with relevant experience is preferred.
Required Qualifications
- Master’s in social work (M.S.W.) with at least 2 years postgraduate professional social service experience (L.C.S.W. preferred)
- A Bachelor’s degree in a mental health field with at least 3 years case management experience working with individuals with chronic/acute health, mental health needs will be considered
- 1 year experience working with unhoused populations required
- Demonstrated history of providing professional, effective case management services
Desired Qualifications
- Currently credentialed to provide supervision to M.S.W. candidates according to the State of Colorado
- Experience with addiction recovery populations

- Must be minimally 21 years of age
- Must have a valid Colorado driver's license
