Job Summary
Lead Case Manager responsible for screening, outreach, and intensive case management for participants in the LEAD diversion program. Conduct outreach on the street and at social service facilities, establish working relationships, and offer supportive services. Provide advocacy and coordination within the criminal justice system, including court appearances and written communication. Transport participants to appointments using agency vehicles and collaborate with LEAD partners, service providers, housing providers, and community organizations to reduce recidivism and improve quality of life for people experiencing homelessness and co-occurring disorders.
Required Qualifications
- Bachelor’s degree in social work, psychology, sociology or other human services-related field required
- Undergraduate degree related to social services or behavioral health with two years’ experience OR five years of relevant experience may substitute for educational requirement
- Understanding of harm reduction
- Street outreach experience is a plus
- Valid Washington State Driver’s License (local travel required)
- Available for on-call response rotation to law enforcement for arrest diversion referrals
- Must pass Washington State Patrol criminal background check and pre-employment drug test
- 10 hour shift, 10:30am-9pm, weekends as needed
