Job Summary
Interview members to assess medical and social determinant gaps, develop individualized care plans, and provide education on conditions and medications. Respond to inquiries, resolve administrative issues, and advocate for members by consulting healthcare providers, arranging appointments, and evaluating progress. Collaborate with interdisciplinary teams to locate resources, coordinate social service plans, and maintain ongoing contact via telehealth and in-person visits. Support behavioral health coordination and substance abuse resources while ensuring adherence to hospital and legal requirements. Maintain empathy and professionalism throughout all member interactions.
Required Qualifications
- High school diploma or GED
- Minimum of 1 year experience in case management, member care, customer service, call center, or member care
- Valid California driver's license
- Valid vehicle insurance
- Excellent communication skills

- Computer skills for documentation, email and chat support
