Job Summary

Conduct comprehensive assessments and develop care plans for high-need youth and families, while engaging in outreach to enroll new members and build rapport. Organize care activities by maintaining regular contact with outside providers, including county substance use disorder and specialty mental health providers, and manage referrals and follow-up to ensure service coordination. Deliver services in person and via telehealth, providing education and identifying support needs for caregivers. Utilize crisis communication and de-escalation techniques, which may involve intervening with clients' behavioral programs through physical management if applicable. Build strong relationships with community providers to identify local supports and resources. This role requires frequent travel up to 90 miles one way for in-home visitations and collaboration across counties.

Required Qualifications

  • Bachelor's degree
  • Associate degree or equivalent + 1 year of applicable work experience
  • High school diploma/GED + 2 years of applicable work experience
  • Applicable experience includes paid or unpaid work with youth, families, or in the social services field
  • At least 21 years of age
  • Driving within the community to meet with clients, attend meetings, or perform job duties at multiple locations within a specific region
  • TB test clearance
  • Fingerprinting clearance
  • Any other state/federal licensing or certification requirements
  • Valid driver's license
  • Clean driving record
  • Insurability through Seneca Family of Agencies' insurance policy
  • Own vehicle
  • Current automobile insurance
  • Must be able to lift 50 lbs

Desired Qualifications

  • Prior case management and community-based experience
  • Lived or family experience in navigating medical and mental health, probation, child welfare, and/or education systems and resources
  • Commitment to advancing racial equity
  • Strong written and oral communication skills
  • Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks
