Job Summary
Lead Care Manager coordinating Enhanced Care Management for high-need youth and families; conducts assessments, creates care plans, coordinates with county and specialty providers, manages referrals and follow-up, provides education, delivers services in person or via telehealth, and performs crisis communication and de-escalation; requires strong communication, organizational skills, commitment to equity, and valid driver's license.
Required Qualifications
- Bachelor's degree OR
- Associate degree or equivalent + 1 year of applicable work experience OR
- High school diploma/GED + 2 years of applicable work experience
- At least 21 years of age
- Driving within the community to meet with clients, attend meetings, or perform job duties at multiple locations within a specific region
- TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
- Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy
- Own vehicle and current automobile insurance
- Preferred: Prior case management and community-based experience preferred
- Lived or family experience in navigating medical and mental health, probation, child welfare, and/or education systems and resources preferred
- Skills of an Ideal Candidate: Commitment to advancing racial equity
- Strong written and oral communication skills
- Strong organizational skills
