Job Summary
Direct support to Gardner Health Services highest risk patients experiencing chronic disease conditions and medical/social barriers. Coordinate care management services for health plan referrals and internal referrals within an interdisciplinary team including primary care providers, RNs, social workers, and health educators. Provide outreach to diverse ethnic and multi-cultural populations while managing care for individuals with multiple needs and low income backgrounds. Full-time, 40 hours/week at the Downtown Resources Center in San Jose. Bilingual English/Spanish preferred.
Required Qualifications
- High School diploma
- Two-years of equivalent work or life experience with low income populations with multiple needs, chronic diseases, or other health conditions
- Demonstrated working knowledge of OSHA and HIPAA guidelines
- Ability to work with a diverse ethnic and multi-cultural population
- Ability to set and meet deadlines
- Ability to take instruction yet work independently and take initiative
- Demonstrates effective problem solving skills
- Computer skills including proficiency in medical practice management systems, internet use, and Microsoft office applications
- Strong communication and conflict resolution skills
- Excellent written and verbal communication skills
- Creative and flexible team player with a sense of humor
- Must be comfortable in a fast-paced environment
Desired Qualifications
- AA or BA degree, preferred
- Medical Assistant Certificate
- Community Health Worker Certificate
- Bilingual English/Spanish, preferred
