Job Summary
Oversee development and implementation of Individual Care Plans while coordinating health benefits and facilitating timely client access to care. Provide direct services via office, telehealth, or field visits, connecting clients to social supports and advocating with healthcare professionals using evidence-based practices like motivational interviewing. Conduct outreach through phone calls, mailings, and field visits to link individuals to the Enhanced Care Management Program, accompanying clients to office visits as needed. Collaborate with hospital staff on Transitional Care Planning and serve as a liaison with external providers to promote client wellness and independence.
Required Qualifications
- Bachelor's degree from an accredited university in a related field
- At least three years of relevant work experience
Desired Qualifications
- Multilingual capabilities
- Prior experience as a community health worker, peer support specialist, medical assistant, or similar role
- Long-time residents of the community with good knowledge of its resources
- Ability to complete training program and ongoing educational requirements as assigned
- Experience with the usage of EMR/EHRs/Care Management software
