Lead Care Manager, Enhanced Care Management

Oversee development and implementation of Individual Care Plans while coordinating health benefits and facilitating client access to care through office-based, telehealth, or field-based services. Connect clients to social supports and advocate with health professionals, utilizing evidence-based practices like motivational interviewing and trauma-informed care. Conduct outreach via phone, mail, and field visits to facilitate program linkage, accompanied clients to office visits as needed, and collaborate with hospital staff on Transitional Care Planning.
