Job Summary
The Lead Care Manager is responsible for providing direct services to individuals referred through the Managed Care plan for Enhanced Care Management. This role acts as the primary contact for clients, coordinating care with various providers and ensuring services are community-based and person-centered. Responsibilities include assisting clients in accessing mental health, educational, and vocational services, as well as advocating for resources to meet client needs. Candidates must have a Master’s or Bachelor’s degree in relevant fields, experience with children/families, and demonstrate strong communication, leadership, and problem-solving skills.
Required Qualifications
- Master’s degree (M.A./M.S.) in psychology, social work, public health or related field with at least 1 year of experience working with children and/or families
- Bachelor’s degree (B.A.) in social services or related field, with at least 2 years of experience working with children and/or families
- Direct service experience working in case management or related experience
Desired Qualifications
- Proficient knowledge of Electronic Health Record
- Proficient knowledge of Microsoft Word
- Proficient knowledge of Outlook
- Proficient knowledge of Excel
- Bilingual Spanish (verbal and written skillset)
Additional Requirements
- Valid California driver’s license
- Current automobile insurance
- Clean driving record acceptable to the organization’s insurance company
- Immediate access to vehicle during work hours
