Job Summary
Service Coordinator to ensure that youth and families receive the right services at the right time by coordinating and managing services between families, DCF, and provider agencies; provide referral information to potential service providers; schedule utilization reviews and ensure appropriate services are provided to DCF children and families; facilitate treatment plan reviews, diagnostic conferences, and discharge planning meetings; monitor service implementation and ensure consistent case coordination; collaborate with providers to ensure accurate and timely documentation, billing, and data entry. The Key Program is a private, non-profit agency delivering trauma-informed, culturally grounded services and works with MA DCF, RI DCYF and NH DCYF. Required Qualifications
- Bachelor’s degree in a related human service or clinical field
- Minimum of 1 year case management or direct care experience
- Must be 21 years of age or older
- Valid state driver’s license and legally registered and insured car for work use
