Lead Agency Service Coordinator, Springfield, MA

Service Coordinator to ensure that youth and families receive the right services at the right time by coordinating and managing services between families, DCF, and provider agencies; provide referral information to potential service providers; schedule utilization reviews and ensure appropriate services are provided to DCF children and families; facilitate treatment plan reviews, diagnostic conferences, and discharge planning meetings; monitor service implementation and ensure consistent case coordination; collaborate with providers to ensure accurate and timely documentation, billing, and data entry. The Key Program is a private, non-profit agency delivering trauma-informed, culturally grounded services and works with MA DCF, RI DCYF and NH DCYF.
