Job Summary
Licensed Clinical Social Worker provides assessment and brief evidence-based behavioral health interventions to adults, older adults, children and families in a primary care setting under PCBH model; collaborates with primary care providers to develop care plans, conducts crisis interventions, maintains EMR documentation, ensures culturally appropriate services, and participates in team meetings, supervision, and quality assurance while supporting integration of services and community resource linkages. Bilingual in English/Spanish or English/Arabic required; CA-licensed LCSW; flexibility for evenings/weekends as needed.
Required Qualifications
- Master’s in Social Work or equivalent from an accredited program
- LCSW license issued by the State of California
- Bilingual in English/Spanish or English/Arabic
- Experience with community mental health preferred
- Knowledge of integrated treatment and care for individuals with co-occurring disorders
- Knowledge of HIPAA privacy and security rules
- Ability to work in a culturally sensitive environment
