Job Summary
Conduct psychosocial assessments, develop care plans, and provide counseling and crisis intervention to individuals and families facing complex challenges. Connect clients to resources while advocating for them and collaborating with multidisciplinary teams to ensure holistic support. Perform crisis management, abuse/neglect identification, and grief/bereavement support, alongside substance abuse screening and health/wellness promotion. Requires a Licensed Clinical Social Worker, CA, with at least two years of clinical experience, AHA CPR certification, and thorough comprehension of OBRA and Title 22. Located at Dignity Health - St. Mary Medical Center in Long Beach, CA.
Required Qualifications
- At least two years of clinical and/or medical social work experience
- Licensed Clinical Social Worker, CA
- AHA Basic Life Support - CPR
- Knowledge of contemporary behavioral and systems theories relevant to health care
- Knowledge of end-of-life dynamics and interventions
- Knowledge of grief and bereavement counseling
- Knowledge of substance abuse identification and interventions
- Knowledge of support for victims of abuse neglect or violence
- Knowledge of community or financial resources for underserved or vulnerable populations
- Knowledge of SSDI SDI and other governmental/county requirements/programs
- Thorough comprehension of OBRA and Title 22 is required in California
