Job Summary
Part-time LCSW-Social Worker Obstetrics providing discharge planning, case management, collaboration with the healthcare team, psychosocial assessments, and community resource coordination across inpatient OB/pediatric units. Responsible for developing individualized treatment plans, leading care conferences for patients with extended stays, documenting and coordinating referrals, ensuring compliance with regulations, and advocating for patients and families to support health outcomes.
Required Qualifications
- Master’s degree in social work
- Licensed clinical social worker in the state of Illinois (LCSW)
- 3 to 5 years work experience in hospital setting or related setting
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
