Job Summary
Conduct psychosocial assessments, develop care plans, and provide counseling and crisis intervention for inpatients, outpatients, and emergency department patients. Collaborate with multidisciplinary teams to integrate medical, social, and resource issues into treatment plans that support patient-centered goals. Perform high-risk screening, facilitate meetings with healthcare teams, and assist with post-acute placements for complex discharges. Establish professional relationships with physicians, case managers, nursing staff, and community agencies to connect clients with financial and other resources. Serve on hospital committees and deliver in-services to staff. Requires a Master's degree, LCSW licensure, and at least two years of clinical social work experience in a medical facility.
Required Qualifications
- At least two-years of clinical and/or medical social work experience
- Masters Degree from a School of Social work accredited by the Council of Social Work Education
- Knowledge of contemporary behavioral and systems theories relevant to health care
- Knowledge of end-of-life dynamics and interventions
- Knowledge of grief and bereavement counseling
- Knowledge of substance abuse identification and interventions
- Knowledge of support for victims of abuse neglect or violence
- Knowledge of community or financial resources for underserved or vulnerable populations
- Knowledge of SSDI SDI and other governmental/county requirements/programs
- Thorough comprehension of OBRA and Title 22
- Licensed Clinical Social Worker (LCSW)
- Excellent customer service and presentation skills
- Strong interpersonal and written communication skills
- Demonstrated ability to apply analytical and problem solving skills
- Demonstrated ability to manage multiple tasks or projects effectively
- Ability to work independently as needed with a high degree of detail orientation
- Ability to work efficiently in a fast-paced environment with changing priorities
