Job Summary
Provide direct inpatient behavioral health social work services as part of an interdisciplinary team. Conduct psychosocial assessments on newly admitted patients and review others’ assessments to ensure goals and an individual plan of care are established. Lead discharge planning, assist with complex discharges, and implement individual or group therapies. Serve as the point of contact for State agencies (e.g., Department of Mental Health and Addiction Services, Probate Court). Lead daily treatment team meetings and provide clinical oversight of social workers and students on the inpatient behavioral health unit. Complete timely documentation in the electronic medical record and participate in insurance reviews when indicated. Collaborate with families, health-system and community professionals on plans of care, and monitor quality indicators specific to inpatient social work.
Required Qualifications
- Master’s degree in social work from an accredited social work program
- Possession of valid clinical license (LCSW) in Connecticut
- The equivalent of three years of post master’s experience as a behavioral health/mental health clinician providing clinical and/or supervisory services.
- Employed as a social worker at Middlesex Health for 2+ years
- Ability to diagnose using DSM 5
- Must have excellent writing and communication skills, be highly organized
- Must be able to work as a part of a team
