Job Summary
LCSW provides integrated behavioral health services within the primary care setting, delivering assessments, brief therapeutic interventions, and care coordination for adolescent and adult patients. Conduct behavioral health assessments and develop individualized treatment and transition plans using evidence-based modalities (CBT, DBT, MI, SBIRT, mindfulness, trauma‐informed care). Manage acute behavioral health concerns including crisis intervention and suicide risk evaluation. Support behavioral health integration in primary care through collaboration, consultation, and care coordination; facilitate referrals to higher levels of care and connect patients to community and hospital resources. Document all care accurately and timely while adhering to regulatory and compliance standards; participate in quality initiatives and model organizational values.
Required Qualifications
- Licensed Clinical Social Worker (LCSW) in CT or NY, depending on work location
- Minimum 3–5 years of clinical experience
- Experience in medical social work, crisis intervention, behavioral health, or integrated care preferred
- Strong clinical judgment, diagnostic skills, communication abilities, and proficiency with evidence-based modalities
