Job Summary
LEAD Case Manager for field-based, long-term intensive case management and outreach to individuals with repeated encounters with law enforcement. Responsibilities include on-call referrals, engagement and screening, assessment of housing, health and social needs, developing individualized service plans, linking participants to harm-reduction services, transporting participants as needed, assisting with applications for federal/state programs, helping with housing and budgeting, advocating with providers and the criminal legal system, coordinating with LEAD partners, and participation in governance meetings. Requires client-centered, trauma-informed practice, collaboration with police and community stakeholders, and capacity to work in high-need populations. Must have a valid driver’s license, reliable vehicle, and ties to local cohorts; vaccination against COVID-19 is required. Educational pathways include bachelor’s degree in related fields or nursing with relevant experience, or associate degree with direct client experience, or equivalent high school and multiple years in human services. This role is based in Swissvale, PA, with field activities across designated cohorts in Allegheny County.
Required Qualifications
- Bachelor’s degree from an accredited college with a major in sociology, social welfare, social work, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, chemical dependency, counseling or education
- A registered nurse, with 2 years’ experience in public health
- An Associate degree from an accredited college with a major in sociology, social welfare, social work, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, chemical dependency, counseling or education; and one year of direct client experience in a health or human service agency
- A high school diploma or GED, and 12 semester credit hours in sociology, social welfare, psychology, gerontology, anthropology, or other social sciences, criminal justice, theology, chemical dependence, nursing, counseling or education; and two years of experience in public or private human services with one year of direct client contact
- A high school diploma or GED, and five years of experience in public or private human services with two years of direct client contact
