Job Summary
Perform case management for Healthy Start clients by assessing educational needs, completing home visits, and initiating tracking procedures for appointments and referrals. Assist clients in accessing community services through education on smoking cessation, childbirth, breastfeeding, and infant development while maintaining updated client files and databases. Provide office coverage to receive phone calls and assist visitors during required hours. This role supports the Healthy Start Program to improve pregnancy outcomes for children and families in Lake County.
Required Qualifications
- A bachelor's degree in social sciences, Social Work, Human Services, or a Health-Related Field such as Health Education, Child and Maternal Health, or Public Health
- Valid Florida driver's license and insurable by the agency's insurance carrier
- Must successfully clear Level II Background Screen
- Must speak fluent Spanish
- Must possess an acceptable driving record and have reliable transportation
- Excellent knowledge of community resources
- Excellent interpersonal and communication skills (verbal and written)
- Excellent computer skills in Microsoft Office Suite (Word, Excel, PowerPoint)
- Effective public speaking skills
- Ability to travel up to 75%
- Ability to adapt to a hybrid work environment as needed
- Ability to lift 10-25 pounds
- Ability to safely operate a motor vehicle
- Ability to work evenings and weekends as needed
- Ability to work in an environment with frequent interruptions and a moderate noise level
- Ability to independently complete job duties within required time frames
- Knowledge of family and systems theory
Desired Qualifications
- 2 years of related experience
- Experience conducting home visits with a similar population
- Knowledge of obstetrics, pediatric health, and health care systems
